Independent Contractor Acknowledgement Form (Form PEDACKN)
In accordance with requirements from the Ohio Public Employees Retirement System (OPERS), we are required to obtain an acknowledgment that you understand, as an independent contractor providing services to the Montgomery County Court of Common Pleas | General Division, you are classified as an independent contractor or another classification other than a public employee and that no contributions will be remitted to OPERS. Exemptions to completing the attached form include “business entities” with five or more employees including corporations, associations, firms, limited liability companies, partnerships, and sole proprietorships (ORC 145.036-145.037). Exempt business entities must sign this letter and return a notarized copy for record keeping purposes.
